Service Description

Please bring your authorization certificate and valid photo identification. Make sure your authorization certificate is signed by your agency BEFORE your appointment.

Cancellation Policy

To cancel or reschedule, please contact us at least 24 hours in advance. The payment is nonrefundable if you cancel your appointment less than 24 hours before it is scheduled or if you do not show up to your scheduled time.
